Hi can anyone please help me with using the same contact form as the one below thanks.

Hi Okinawa,

Some months ago i found a free form builder program. I found easy to build contact forms with it. All You have to do is to choose the desired form type and name tham. The program will generate the php code and an additional css file. You can use iframe to place the forms into html files.

I would recommend putting catcha in any email form that is open to the public.
You would be surprised on how many people and bots search for forms of the such.
Then mass mail using your server.
