Really nice html, but as far as i know html is not able to send an email alone, would you be so kind to extend it with the required php script?

PHPMailer is a very powerful PHP class for email facilty. This will definitely help you for PHP part. As long as your html side is working well, you have to customized your php code according to you html form. Anyway, there are some examples in that site. You can have some idea out of those.
